To place an order for a signature matrix, you will need to:

1. Complete a signature matrix order form which includes 2 signature samples inside the ovals.
2. Choose the signature that you prefer.
3. Choose turnaround time.  
4. Provide billing and shipping information.
5. Fax and/or mail to Automated Signature Technology.  If faxing, please mail the original, marking "Confirmation Only".
